Olympia Vision Clinic Financial Policy

We will be happy to bill your medical and/or vision insurance if you have provided us with your most current insurance card and information.  If you have any questions about your insurance coverage, please contact your insurance representative.  Most insurance policies pay only a portion of your total charges.  You understand that all benefits quoted to you are not a guarantee of payment by your insurance company and that final determination can only be made when the claim is processed.  Accounts 90 days old are subject to collection fees.  There will be a service charge on all returned checks.  You authorize the release of any medical or other information necessary to process insurance claims.  You also request payment of government benefits to the party who accepts assignment.  You authorize payment of medical benefits to the physician or supplier for the services rendered.  

You understand (even if you do not have insurance) that you will be financially responsible for payment of all charges incurred for services from Olympia Vision Clinic. 

Olympia Vision Clinic Refund Policy

We are committed to you, our customer, being satisfied with your purchase and will work with you to ensure our products meets with your needs.   Refunds are made on a case by case basis.
